What does a Title 5 Inspection include?

• Uncovering the septic tank and distribution box by digging

• Use of an excavator is necessary when the tank and D-box are deeper than 3'

• Contacting the town or city for as-built, water use, and pumping records

• Locating all wells and wetlands within proximity of the system

• Uploading the MassDEP Title 5 form to the Board of Health

Title 5 inspections are official, state-authorized inspections and the results are uploaded to the Board of Health.

Soil Evaluations

Title 5 soil evaluations determine the location for the installation of a septic system. Soil evaluations determine the water table elevation as well as the soil profile.

Unofficial inspections can be performed to inform the homeowner of the condition of the system
